The questionnaire covers what a dive computer cannot record. Four steps ask: Feel, The dive, You and Next 24h, symptom checks first.
The symptom checks
Nine checks, each yes or no, with None of these — I feel fine as an explicit answer:
- Any unusual joint or limb pain (elbows, shoulders, knees)?
- Any skin itching, rash, or a mottled / marbled appearance?
- Unusual, disproportionate tiredness after this dive?
- Any numbness, tingling, or “pins and needles”?
- Any muscle weakness, clumsiness, or difficulty moving a limb?
- Any dizziness, vertigo, ringing ears, or hearing change?
- Any confusion, trouble concentrating, or slurred speech?
- Any cough, chest pain behind the breastbone, or breathlessness?
- Any band-like torso pain, trouble urinating, or leg weakness?
The last six are marked Serious. Report anything and a further question appears: how long after surfacing it began. Joint pain, skin signs and fatigue also ask how severe: Barely noticeable, Mild, Moderate, Severe, Disabling.
If you report one
A banner appears and stays, listing what you reported and when it began. A serious symptom gives, verbatim:
You reported a symptom that can indicate decompression illness. Do not dismiss it, even if it seems mild or is fading. Breathe 100% oxygen, stay still, and contact a dive physician or DAN now.

A milder one gives:
You reported a possible post-dive symptom. Keep monitoring how you feel. If it worsens or a neurological sign appears, breathe 100% oxygen and seek dive-medical advice.
What else it asks
- The dive: ascent control, stops, gas switches, sawtoothing, setpoint stability, CNS, whether ppO₂ passed 1.6 bar, OTU, oxygen-toxicity signs, warmth, exertion.
- You: how you felt going in, rest, hydration, PFO, previous decompression sickness, alcohol in the twelve hours before, dives today, consecutive days, reverse profiles.
- Next 24h: hours until flying or driving to altitude, strenuous exercise, hot shower, sauna or hot tub.
Most answers are five-point scales in plain language. Questions your dive data answers appear instead under Answered from your dive data, each with a Change control.
The scored answers give a decompression-stress reading out of 100: Conservative below 25, Moderate 25 to 54, Provocative 55 and above. A symptom flag is listed first.
